For a national non-profit association interested in radically changing the way communities provide comprehensive services to seniors, Coray Gurnitz facilitated community involvement meetings of healthcare providers in six states. In each community, the group began developing a new system of care that more effectively responds to the needs of older adults with acute health issues and nutritional deficits.
To help these communities transform their present system of care from disparate and separate care systems to a holistic, integrated continuum of care, we convened meetings with local hospitals and other community healthcare staff to pilot a new coordinated system of care to connect hospital discharged seniors who are nutritionally at risk.
Coray Gurnitz used an “appreciative” approach in these community engagement meetings to re-design the systems and processes—participants imagined the new system as if it were already in place and working perfectly. With this “dream” articulated, we helped the key stakeholders in the system identify all the relevant parties to the new system, as well as the plan for designing and implementing the details of how the system would actually work.
